Certificate of Eligibility for:
o 1st Level position (Non-Professional - Clerical, Trades, Crafts & Custodial Services)
o 2nd Level position (Professional and Technical)
o 3rd Level position (Career Executive Service)
Certificates (Trainings/Seminars attended)
Perormance rating in the last rating period (if applicable)
Medical Certificate

HOW TO AVAIL OF THE SERVICE

FOLLOW THESE STEPS IT WILL TAKE YOU PLEASE APPROACH
1. Submit application letter with the requirements. 2 to 5 minutes HRMO Staff
2. Subject yourself to interview; and examination, if necessary. 30 to 60 minutes Municipal Personnel Officer Personnel Selection Board
3. If selected, fill out and submit duly accomplished Personal Data Sheet with Medical Certificate. 20 to 30 minutes HRMO Staff
4. Receive Municipal Appointment paper and/or Civil Service Commission (CSC) approved Appointment, if applicable. 15 to 20 minutes Municipal Personnel Officer HRMO Staff
5. Report to work. Respective Department Head

APPLYING FOR ON-THE-JOB TRAINING (OJT)

ABOUT THE SERVICE

This provides assistance to student-trainees to gain actual experience and apply their skills and capabilities in office work.

REQUIREMENT(S)

  • Recommendation letter from the School
  • Resumé  or   Bio-data

HOW TO AVAIL OF THE SERVICE

FOLLOW THESE STEPS IT WILL TAKE YOU PLEASE APPROACH
1. Submit recommendation letter. 5 minutes HRMO Staff
2. Subject yourself to an interview and be advised of assigned office/department. 15 to 30 minutes Municipal Administrator
Municipal Personnel Officer
3. Report to the concerned office/department head. 5 to 10 minutes Concerned Department Head/ Chief of Office
4. Report to work as per office schedule. Depends on the required number of hours Concerned Department Head/ Chief of Office
5. Present Daily Time Record for certification and submit Rating Sheet for evaluation and grading/assessment. 10 to 15 minutes Concerned Department Head/ Chief of Office
6. Submit duly certified Daily Time Record for the issuance of Certificate of Completion. 20 to 30 minutes HRMO Staff
7. Claim Certificate of Completion. 5 to 10 minutes HRMO Staff

1. Secure and submit appropriately filled-out HRMO request slip. 5 to 10 minutes HRMO Staff
2. Pick-up requested document on the following day after submitting the HRMO request slip. 5 to 10 minutes HRMO Staff

Allow 24 hours processing time for the abovementioned requested document(s).

Processing time for Monetized Leave and/or Terminal Leave request is 2 to 3 days.
