ABOUT THE SERVICE
This provides the timely and delayed registration of birth, marriage and death.
National Statistics Office (NSO) Administrative Order No. 1 Series of 1993 states the following:
The birth of a child shall be registered within (30) days from the time of birth; registration of marriage is within 15 days following the solemnization of marriage while in marriage exempt from license requirement, the prescribed period is thirty (30) days; and registration of death is within thirty (30) days from the time of death.

Four (4) copies of the duly accomplished Certificate of Death | Four (4) copies of Certificate of death which must be accomplished correctly and completely Notarized Affidavit for delayed registration which shall be executed by the hospital/clinic or similar institution or if the person died elsewhere, by the attendant at death. In default of the hospital/clinic administrator or attendant at death, the affidavit shall be executed by any of the nearest relative of the deceased, or by any person having legal charge of the deceased when he was still alive. The affidavit shall state among other things, the name of the deceased, the facts of his death, the date and place of burial or cremation and the circumstances why the death was not reported for registration within thirty (30) days after death. Note: The license shall be valid in any part of the Philippines for a period of one hundred twenty (120) days from the date of issue, and shall be deemed automatically cancelled at the expiration of said period if the contracting parties have not made use of it. This involves the legitimation of birth of children born out of wedlock.

ABOUT THE SERVICE
This involves the registration of foundling.
Foundling is a deserted or abandoned infant or a child found, with unknown parents, guardians or relatives and/or a child committed in an orphanage or charitable or similar institution.

AVAILING OF THE LEGAL MEANS TO USE FATHER’S SURNAME FOR AN ILLEGITIMATE CHILD
ABOUT THE SERVICE
This involves the use of father’s surname for an illegitimate child in accordance with Republic Act No. 9255 also known as “Authority to Use the Surname of the Father (AUSF) for Illegitimate Child”.
